Registered Nurse- Patient Aligned Care Team (PACT)

The RN Care Manager triages Veterans who walk or call into the CBOC for health care related issues and coordinates care as appropriate to their needs. The RN Care Manager provides cross coverage to other areas of the CBOC such as lab, tele-health, and other PACT teams as necessary. The primary duties for the RN- PACT position include but are not limited to:
Screens and coordinates care for Walk Ins and clinical phone calls (right time, right place, right person) Nursing Telephone Visits Group Visits/Shared Med Appts. Pt./family education such as new patient orientation, mentor/precept nurse trainees, patient health education and coaching. Pertinent Clinical Reminders Follow-up calls including lab review per policy Responds to triaged messages and e-mail with consultants Assists with Prevent. Care & stable chron. disease management based on virtual and face to face ongoing review of patient and Dashboards Utilizes motivational interviewing to guide pts to establish goals Coordinates care especially for higher risk pts as needed for procedures, transitions, pt./family teaching, etc Initiates appropriate Protocols or alerts Provider for referrals (CCHT, HBPC, hospice/palliative, etc) collaborating with team and internal and external services and by contacting/assessing patients' needs 48 hour post-discharge call when discharged from Inpt./ER Travel between the CBOCs (Watertown and Aberdeen SD) is required for coverage needs as necessary to ensure continuity of operations. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d). Preferred education, knowledge, and skills:
2 to 3 years of experience as a registered nurse. Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. The work requires some standing, walking, bending, lifting, and carrying of light items. The work requires agility and dexterity and may require heavy lifting of equipment or patients. In many situations, the duration of the activity (such as most of a workday) contributes to the arduous nature of the job. Duties require extensive periods of standing and walking. Working with patients requires regular and recurring bending, lifting, stooping, stretching, and similar activities.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
