Registered Nurse- Patient Aligned Care Teams

The PACT RN responsibilities include care coordination, effective disease and chronic care management along with disease prevention in areas of population management and preventive care. The primary duties for the RN- PACT position include but are not limited to:
Supports the medical center's mission and core values. Provide prescribed medical treatment and personal care services to ill and injured Veterans while following the PACT model of care. Interacts with Veterans and provides services in a manner that supports personalized, proactive and patient centered care. Provides efficient care triaging patients according to established protocols, population health and PACT Model of Care. Documenting observations, assessments, and changes in patient's condition. Collaborating with health team members to facilitate positive patient care outcomes. Providing a full range of nursing care to patients with a variety of physical and/or behavior problems. Educating patients about healthy practices and treatment modalities. Communicating effectively with internal and external customers, including, but not limited to:
demonstrating professional speaking and writing abilities and documenting interactions in a responsible, accountable and ethical manner that will facilitate quality review and promote effective care management. Complies with health, safety and environmental rules and procedures. Performs work in a manner that enhances the safety of the work environment. Ensures strict adherence to federal, state, and local codes concerning health, safety and the environment as applicable within the performance of his/her duties. Assist each patient to identify and meet basic health needs and improve his/her health status to the individual's maximum potential. NOTE:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d). Note:

BLS/ACLS Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. The work requires some standing, walking, bending, lifting, and carrying of light items. The work requires agility and dexterity and may require heavy lifting of equipment or patients. In many situations, the duration of the activity (such as most of a workday) contributes to the arduous nature of the job. Duties require extensive periods of standing and walking. Working with patients requires regular and recurring bending, lifting, stooping, stretching, and similar activities.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
